1. Acknowledgment of Risks
I understand that my child’s participation in Youth Athletic Advantage (YAA Toronto) programs, classes, and activities involves physical exercise and athletic training, including but not limited to running, jumping, agility drills, strength exercises, obstacle courses, and use of training equipment. I acknowledge that participation carries certain inherent risks, including the risk of injury (such as strains, sprains, fractures, or other injuries), and in rare cases, more serious harm.
2. Voluntary Participation
I certify that my child is in good physical condition and has no known health concerns that would prevent safe participation in these activities. I voluntarily allow my child to participate in YAA Toronto programs and accept full responsibility for their participation.
3. Release of Liability
In consideration for my child being permitted to participate in Youth Athletic Advantage programs, I, on behalf of myself, my child, and our heirs, executors, and administrators, hereby waive, release, and discharge Tanuki Martial Arts, its owners, officers, employees, agents, and representatives from any and all claims, liabilities, demands, or actions arising out of or related to my child’s participation, whether caused by negligence or otherwise, to the fullest extent permitted by law.
4. Medical Consent
In the event of an injury or medical emergency involving my child, I authorize the staff of Youth Athletic Advantage / Tanuki Martial Arts to obtain medical care as deemed necessary, including transportation to a hospital or medical facility. I accept full responsibility for any resulting medical expenses.
5. Media / Photo Release
Photographs and videos of participants may be taken during Youth Athletic Advantage / Tanuki Martial Arts programs, classes, or events, and may be used for promotional purposes including social media, website, flyers, or other marketing materials, unless otherwise stated by the parent/guardian.
6. Governing Law
This waiver shall be governed by and interpreted in accordance with the laws of the Province of Ontario.
